Nondiscrimination Policy

Winnebago Comprehensive Healthcare System (“WCHS”), including Twelve Clans Unity Hospital and Winnebago Public Health Department, complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or gender.

Discrimination is Against the Law

WCHS does not exclude people or treat them differently because of race, color, national origin, age, disability, or gender.

NOTE: Exclusion from benefits limited by Federal law to individuals of a particular race, color or national origin is not discrimination (for example, funds under the Purchased/Referred Care program used to supplement and complement other health care resources available to eligible American Indians and Alaska Natives). 42 CFR § 80.3(d) 

WCHS Provides

Free aids and services to people with disabilities to communicate effectively with us, such as:
  • Qualified sign language interpreters
  • Written information in other formats (e.g., accessible electronic formats)

Free language services to people whose primary language is not English, such as:

  • Qualified interpreters
  • Information written in other languages

Failure to Provide

If you believe that WCHS has failed to provide these services, or discriminated in another way on the basis of race, color, national origin, age, disability, or gender, you can:

  1. File a grievance with our Patient Advocate, Tel: 402-745-3950. You may file a grievance in person or by mail. If you need help filing a grievance, the Patient Advocate, is available to help you.
  2. You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through the Office for Civil Rights Complaint Portal, available at https://ocrportal.hhs.gov/ocr/smartscreen/main.jsf , or by mail or phone at:
