Winnebago Public Health Department

Elevating Population Health

For the past 50+ years, the Winnebago Public Health Department, one of the two divisions of the Winnebago Comprehensive Healthcare System has been providing essential public health services to the community of Winnebago, NE. Building on proud traditions with an eye to the future, WCHS' Public Health Department remains committed to improving population health while adapting as the health and wellness  needs of all relatives change.

Understanding Public Health

The conditions around you where you are born, grow, work, live and age are called social determinants of health. They can have a wide-ranging effect on your health risks and outcomes. The public health framework seeks to highlight ways to remove obstacles, and systematic, or structural barriers including poverty, racism, gender discrimination, social determinants of health, and other forms of oppression that have resulted in health inequities. The WPHD has evolved to focus on the Indigenous social determinants of health as they understand the importance of language and cultural practices.Winnebago Public Health Department uses tools like our Community Health Assessment and Tribal Community Health Improvement Plan to empower health professionals to identify inequities and to work to introduce policies, systems, and services that improve health in our community.

Every five years, the Winnebago Public Health Department conducts a Community Health Assessment (CHA). This Tribal health assessment identifies key health needs and issues through systematic, comprehensive data collection, and analysis. If you would like to see the results of the latest CHA, the full report is available for public consumption in any of the Winnebago Public Health Department Director's offices.

When the Community Health Assessment is complete, the Winnebago Public Health Department uses those results and key takeaways to create a Tribal Community Health Improvement Plan (TCHIP). This plan establishes goals and comprehensive initiatives that guide our Winnebago Public Health Department over the next 5 years.
