Escorting You to Appointments
Our CHRs corrdinate with you to book rides to and from WCHS and other healthcare facilities in the surrounding region. Providing one of WCHS's two options for non-emergency medical transportation (NEMT) services.
When utilizing Purchased Referred Care to schedule appointment with healthcare facilites outside of the Winnebago Comprehensive Healthcare System - it is important to remember to schedule your ride from WCHS seperately. If any changes are made in your outside appointment, we encourage you to be a good relative and call to cancel or reschedule your ride with us. This communication is critical to allowing us to maximize our availablity.

The Passenger Code of Conduct is a summary of non-negotiable passenger expectations. As such, our employees reserve the right to refuse passage, call WCHS security or law enforcement as needed.

Home Visit Services:
The Community Health Reps are here to support, advocate, connect, and empower you at every step on your health journey including:
- delivering food and medication for the elderly, disabled, and homebound in our community
- making home visits and wellness checks to ensure social and environmental needs are met.
- escorting patients on their medical appointments to help support and advocate.
- navigating, connecting, and recommending all WCHS resources.
- providing scheduled rides to and from medical appointments
A team focused on meeting relatives where they are to better equip and/or allow better access of services to result in more equitable healthcare. By better equipping our community member with convenient resources like this, we are ensuring our relatives are able to take advantage of the many preventive healthcare services available to them.
