Public Health Nursing focuses on improving the overall health and well-being of communities through education, prevention, and outreach. This team of nurses promote health, prevent disease, and address health disparities by providing services such as screenings, immunizations, and health education. By assessing community needs and offering targeted interventions, the Public Health Nurses at WCHS aim to create healthier environments and empower individuals to take control of their health.
Population Health
The Winnebago Public Health Department began in the 1970s with a team of two Public Health Nurses (PHNs), throughout the years the team has grown in programs and responsibilities but the focus on prevention, education, and population health has never wavered.
Public Health Nursing, also known as community health nursing, aims to prevent disease, promote wellness, and improve healthcare access for all. By combining expertise in nursing, social sciences, and public health, these professionals assess community health needs, identifying individuals and families who may benefit from targeted health interventions. Through education, preventive care, and early intervention, public health nurses reduce infectious disease transmission, manage chronic conditions, and address health disparities. Their work fosters healthier communities, supports the local underserved population, and promotes sustainable, easily accessible healthcare by enhancing overall well-being across the Winnebago Community.
Collaboration
PHNs work hand and hand with the community and the Public Health Department as a whole to develop plans to address these needs, taking into account available resources and activities that contribute to health. Together, they collaborate with departments throughout WCHS to encourage lifestyle changes and educate our patients communities on healthy living.
