Advanced Primary Care is a cornerstone of our Winnebago Care Model, designed to empower our patient community by providing seamless, comprehensive care at every stage of their health journey.

You are a whole person. All your healthcare needs should be centered around you. At WCHS, we are building an Advanced Primary Care that centers on you. We are working to centralize your primary health services to improve access, communication, & collaboration with the intention of creating better health support for every relative, every time. 

What Does that Mean:

Dedicated Care Team

We designate an integrated primary care team that will be dedicated to you. They will be your trusted advocates helping you navigate our health system and take charge of your health. 

Central Registration 

With the help of our Master Facilities Plan, we will be combining our clinical services to one central location for ease of access. Today, when you arrive at WCHS you will navigate to a centralized registration desk for check in. We are striving to make the right health choices, the easy choices. 

One WinnHealth Record

We have moved to one modern electronic health record for storing all patient information from family medical histories to notes from each visit. Your WinnHealth record will be available in the WinnHealth Portal enabling you to easily manage your care anywhere you are.  

Bring for Your Visit 

  • WCHS will be confirming your appointment and remind you that it is coming with text, call and email communications. 
  • Bring your enrollment ID or Certificate Degree of Indian Blood, Insurance Card, a valid photo ID
  • Bring any health questions or concerns that you have.  

When you arrive you will be asked to sign forms like: consent to treats, updating your family history, and other relevant information and then directed to wait in the correct waiting room to be called. As soon as our care team is able you will be called and you will be escorted back to a patient room. The nurse will update your chart and do some initial evaluations and then you are ready to meet with your provider.
